USMLE STEP 2 CS RELAUNCH – 3 KEY PRIORITIES TO BE TESTED
The USMLE has given the following announcement that the USMLE step 2 CS exam will be returning in early 2021. Over the summer they collected an initial community feedback to help for the exam future development. This involved focus group discussions and a fairly large survey from stake holders which included State Board members, faculty of medical schools, medical students, residents, international medical graduates and residency program directors. They plan to retain the valuable inputs and address any concerns wherever feasible. Valued assets of stake holders on step 2 CS exam were on reliable scoring, standardization of SP performance across sites, clinical skillsets being assessed and how relevant they are to clinical practice, professional smooth operation of test administrations. There was a broad agreement among all stake holder group that this type of assessment is relevant and important. The concerns were on the limited number of test sites, examinees travel away from home, especially the impact for IMGs outside the country to test at one of the 6 clinical sites across the US. Other challenges were the cost of the exam, perception of the artificial nature of the Standardized patient interaction and the patient note. This announcement has been much awaited by students applying for the 2022 Match.
3 Key priorities for the New Step 2 CS exam in 2021
1. Can an examinee conduct an effective clinical interview? Can they obtain the right information?
2. Can they communicate that information effectively to patients and other members of the health care team
3. Can they reason effectively so they can take the most appropriate steps in patient care.
USMLE REVITALIZES CLINICAL SKILLS ASSESSMENT IN 3 PHASES
3 Phases of the relaunch of step 2 CS are listed below
Discovery – this phase has been completed which includes the licensing needs assessment, prioritization stakeholder input)
Implementation – Training, pretest, pilot studies will be conduct in this phase.
They conceded to the fact they are very much aware of the 2022 NRMP match timelines, ECFMG certifications and Step 3 timelines and will relaunch Step 2 CS at a time that balances these many factors.
Overall the Modified assessment will be delivered via remote online delivery platform with examinees interacting with standardized patients over a video. This is provide a safe and convenient platform that promotes both examinees and staff safety.

- Empathy Preparation: A Communication Instructor explains how to display Clinical
- Empathy. You will learn the Basic Understanding of Empathy for both Adult and Pediatric encounters. After the discuss you will practice your skills.
- Full Day Practice Examination with Several Standardized Patients Encounters in a medical-clinic environment. Each examination room is fully-equipped with all medical instruments, a stool, and an examination table – to prepare you for the actual exam.
- Computer generated announcements with time lines similar to what you will experience in the actual CS exam.
- Examination Proctor – to oversee the exam.
- Video monitored exam by Staff Physicians.
- Physician-evaluated and graded Patient Notes on most of your cases
- Multiple Unique Encounters – including phone encounters
- Encounters with Challenging Emotions and Questions
- Standardized Patient will provide you with 1-on-1 individualized feedback

- Data Gathering and Data Interpretation: The morning is devoted to understanding how to approach various cases and what Physical Examination should be completed for each case.
- Review Clinical Knowledge related to Patient encounters.
- Review of Complex Cases: Blank Doorway,Rape, Domestic Violence, Child Abuse, Depression, Hallucinations etc.
- Practice Communication Skills from specific areas of your weaknesses with Communication Instructor and/or Standardized Patients. You are given specific scenarios that may pose a challenge for you in the actual examination and you practice the Interview, Closure, Counseling, and Challenging questions.
- Focused and Individualized assignments are given based on deficiencies seen during encounters and examination performance.

- Clinical Empathy Skills practiced with various scenarios.
- After class study instructions – given by our Physicians.

- Student-Instructor Wrap-up – at the conclusion of each Workshop each student is given direct access to the Physician Instructor.
